How to apply for Affiliation or Designated Affiliation:
- Read the general information for new applicants.
- Read the Ethical Values and Standards, the Terms of Reference and the Scope of Designation.
- Download the relevant application form.
- Fill out the application form accordingly and please do not omit any fields.
- Return the completed, signed and scanned form to the CPSC office by e-mail.
- Include the proof of payment of the applicable fees and a copy of your ID.
